Neck Pain from Poor Posture: Physiotherapy, Treatment & Prevention

Neck pain is a common problem that can affect people of all ages. It may develop gradually with prolonged desk work, studying, driving or phone use, or occur following an injury or a change in physical activity. Neck pain from poor posture is often discussed, particularly when someone spends long periods sitting at a desk or looking down at a screen. However, the relationship between posture and neck pain is more complex than simply having a “good” or “bad” posture.

Current evidence suggests that posture may be one contributing factor among many. Physical activity levels, muscle capacity, work demands, sleep, stress, previous injuries and the amount of time spent in one position may also influence neck pain. An evidence-based approach to neck pain treatment therefore focuses on improving movement and physical capacity rather than trying to maintain one “perfect” posture throughout the day.

What Is “Poor Posture” and How Can It Affect the Neck?

When people refer to poor posture, they often mean a forward head position, rounded shoulders or a slouched upper back. Forward head posture occurs when the head sits further forward relative to the shoulders and upper body and can be particularly noticeable during prolonged computer or phone use.

Maintaining any position for a prolonged period may increase the demand placed on muscles and joints around the neck and upper back. This can contribute to feelings of stiffness, fatigue or discomfort, particularly if someone spends several hours each day in the same position.

However, having a particular posture does not necessarily mean that someone will develop neck pain. Research examining the relationship between forward head posture and neck pain has produced mixed results. A systematic review and meta-analysis found that the relationship between forward head posture and neck pain remains controversial, with differences between people with and without neck pain not consistently demonstrating that forward head posture itself causes symptoms.

A 2023 systematic review similarly found that some measures of head and neck posture were associated with neck pain and disability, while other commonly used measures were not consistently related to pain. This supports the idea that posture should be considered alongside the broader physical, psychological and lifestyle factors that can influence neck pain.

Common Symptoms of Neck Pain

Neck pain associated with prolonged positions or activity may present in several ways, including:

  • Aching or stiffness around the neck and upper shoulders

  • Reduced neck movement

  • Discomfort after prolonged computer or phone use

  • Muscle fatigue or tightness

  • Pain around the upper back or between the shoulder blades

  • Headaches that may be associated with the neck

  • Symptoms that improve after changing position or moving around

Pain extending into the shoulder or arm can also occur. However, significant weakness, numbness, pins and needles, rapidly worsening symptoms, severe symptoms following trauma, unexplained weight loss or fever may warrant assessment by an appropriate healthcare professional.

How Can Physiotherapy Help With Neck Pain?

Physiotherapy for neck pain can help identify factors that may be contributing to an individual's symptoms. A physiotherapy assessment may include assessing neck movement, strength, mobility, muscle function, work demands, physical activity and the person's goals.

Treatment can then be tailored to the individual's presentation rather than focusing solely on correcting posture. Depending on the individual, neck pain physiotherapy may include exercise, strengthening, mobility work, education, ergonomic advice, movement strategies and manual therapy.

Exercise and Strengthening for Neck Pain

Exercise is an important component of physiotherapy treatment for many people with non-specific neck pain. Depending on the individual's presentation, a program may include neck strengthening, deep cervical muscle exercises, shoulder and upper-back strengthening, mobility exercises and general physical activity.

A 2023 systematic review and meta-analysis of 21 studies found that exercise therapy was associated with improvements in pain and disability in people with chronic non-specific neck pain. The authors noted that much of the available evidence focused on chronic rather than acute or subacute neck pain.

Exercises targeting the deep cervical muscles may also be considered for some people. Research suggests that these exercises can improve pain and disability in people with non-specific neck pain, although there is not necessarily one type of exercise that is superior for everyone.

The most appropriate neck pain exercise program will depend on the individual's symptoms, physical capacity, preferences and goals.

Movement and Ergonomic Strategies for Neck Pain

Rather than attempting to maintain a perfectly upright posture throughout the day, it can be useful to vary positions and incorporate regular movement.

For someone working at a computer, strategies may include:

  • Taking regular movement breaks

  • Alternating between sitting and standing where practical

  • Positioning the monitor at a comfortable height

  • Keeping frequently used items within easy reach

  • Varying hand and device positions when using a phone

  • Incorporating simple neck, shoulder and upper-back movements throughout the day

These strategies are intended to reduce prolonged periods in one position and make work and daily activities more comfortable, rather than suggesting that a particular posture is inherently harmful.

Manual Therapy for Neck Pain

Depending on the individual's assessment and preferences, physiotherapy may also include manual techniques such as joint mobilisation or soft-tissue treatment.

Research suggests that manual therapy may provide short-term improvements in pain and movement for some people with neck pain. It is generally considered most useful when incorporated into a broader active rehabilitation program rather than being relied upon as the sole treatment approach.

Education and Self-Management for Neck Pain

Education is another important component of physiotherapy. Understanding that neck pain does not necessarily indicate damage to the neck may help people remain active and avoid unnecessary fear of movement.

Current clinical practice guidance for non-specific neck pain supports an active approach that includes self-management, with exercise therapy being recommended for chronic neck pain.

A physiotherapist can provide guidance on gradually returning to normal activities, managing aggravating factors and developing strategies that fit within a person's work and lifestyle.

Can Physiotherapy Improve Posture?

Exercise may improve certain measures of posture. However, improving posture does not necessarily need to be the primary goal of treatment.

A 2018 systematic review and meta-analysis found that therapeutic exercise could improve the craniovertebral angle, a measurement commonly used to assess forward head posture. The review also found evidence of improvements in neck pain, although the relationship between changes in posture and improvements in pain was not established.

More recent research has continued to investigate this relationship. A 2026 systematic review and meta-analysis found that neck-focused and combined neck-and-thoracic exercise programs were associated with improvements in some measures of posture and neck-related disability in people with forward head posture. However, the certainty of evidence was rated low to very low, meaning the findings should be interpreted cautiously.

This reinforces an important principle: physiotherapy for neck pain does not need to be about forcing the body into a “perfect” posture. It can instead focus on improving movement, strength, physical capacity and confidence with everyday activities.

How to Prevent Neck Pain

Neck pain cannot always be prevented, but several strategies may reduce the likelihood of developing persistent or recurrent symptoms.

Keep moving: Regular physical activity and exercise can help maintain strength and physical capacity.

Vary your position: Even a comfortable posture may become uncomfortable when maintained for prolonged periods. Regular movement and position changes may be useful.

Consider your workstation: A comfortable desk, chair and monitor setup may make prolonged work easier to tolerate.

Build strength gradually: Strengthening the neck, shoulders and upper back may improve tolerance to everyday activities.

Manage screen use: When using phones and tablets, try to vary your position and incorporate regular breaks.

Consider sleep and stress: Neck pain can be influenced by factors beyond posture, including sleep, stress and overall wellbeing.

Research also suggests that exercise may have a preventative role. A 2023 systematic review and meta-analysis of randomised controlled trials found that exercise interventions were associated with a reduced risk of developing neck pain in adults who were initially free from neck pain.

Take-Home Message: Managing Neck Pain

Posture may play a role in neck pain, but there is no single “perfect” posture that everyone needs to maintain. Having a forward head position or rounded shoulders does not automatically mean that someone will develop neck pain.

A broader approach may be more useful: keep moving, build strength and physical capacity, vary your positions, consider your work and screen habits, and address the individual factors that may be contributing to your symptoms.

A physiotherapy assessment for neck pain may help identify these contributing factors and guide an individualised management plan. Depending on the person's presentation, this may include exercise, education, movement strategies, ergonomic advice and manual therapy.
